What are therapeutic peptides?

Therapeutic peptides are peptide-based drugs approved by regulators (the FDA, EMA and others) to treat specific conditions. There are more than 100 approved worldwide, and the category is booming. They work by copying or blocking the body’s own hormone signals with great precision, which often means strong effects with fewer off-target problems than small-molecule drugs — but also real side effects that need medical supervision.

The GLP-1 moment

Semaglutide and tirzepatide are engineered peptides that mimic gut hormones (GLP-1, and for tirzepatide also GIP). They lower blood sugar, slow stomach emptying and reduce appetite — which is why they’re transformative for type-2 diabetes and obesity. They’ve driven a wave of public interest in peptides, and unfortunately a parallel wave of grey-market “research” copies (covered in research peptides).

How they work

Most therapeutic peptides are receptor agonists or antagonists: they either switch on a receptor (like GLP-1 telling the pancreas to release insulin and the brain to signal fullness) or block one. Because peptides are fragile in the gut, many are injected — though clever formulation has produced oral versions (like oral semaglutide) and nasal sprays for a few.
